Where do I upload my AU health documentation?

 3 MIN. READ

We’ve got you covered when it comes to healthcare. AU’s Health Services team is committed to providing free comprehensive services to students and as part of the health benefits for eligible faculty and staff. 👩🏽‍⚕️👨🏼‍⚕️

These past few years have taught us a lot about being proactive when it comes to helping our campus community stay healthy, and that includes having updated health documentation on file for our students and staff.

Please upload your Report of Health History for our team, along with other required medical forms, and learn more about our efforts to stay safe and healthy as a campus community since the COVID-19 pandemic.

An updated record of your immunizations should be submitted as part of the housing application. This documentation must be submitted before students can move into the residence halls. 📑 🩺

Our Ravens are ready for the return of student activities and meeting together in person, but we must work together to do so safely. Here’s to a healthy community and a healthy you!

God Bless, Go Ravens!
