Entrance Health Requirements
All undergraduate students entering Anderson University are required to submit a Report of Health History to the Director of Health Services by July 15. Health requirements for students at Anderson University exist primarily to protect students from diseases, which are infectious, and to secure medical information concerning students should they need assistance from the Health Services Department. All undergraduate students must meet the immunization requirements and must complete all pages of the Report of Health History including the physical examination. Housing assignment may be withheld until completed Health History and immunization record has been submitted to Health Services.
All graduate students must meet the immunization requirements and must complete page 1, 2, and 4 of the Report of Health History Form but are not required to have an exam, unless they are enrolled in any undergraduate courses, in which case they will need to meet the same requirements as any other undergraduate student.
Students who are not residents or citizens of the United States must have a TB skin test performed in the United States. This may be done at Health Services for a fee of $5.
